The Business Lending & Development Services Department is making every effort to
provide you forms that can be used easily. Some forms can be filled out on your computer directly, others require a printout and a manual completion. Not all forms are designed
by the PADD; some forms (i.e. starting with AD / IRS / RD / SBA / SF) are forms from governmental agencies that are necessary for the application process. The PADD has no control
over the design and complexity of these forms. All forms that are designed by the PADD have to fulfill certain state and federal requirements.
