1.  HUMAN SERVICES DIVISION: Area Agency on Aging & Independent Living
     Long-Term Care Ombudsman

The Purchase Area Development District is seeking a qualified individual to provide Long-Term Care (LTC) Ombudsman services in the eight county Purchase area.  Primary responsibilities include identifying, investigating, resolving, or attempting to resolve complaints made by, or on behalf of, LTC residents; assure residents’ rights are upheld; promote community involvement in the program; facilitate an active volunteer program.

Candidates must possess a bachelor’s degree in health or human services from an accredited college or university with one year experience in health or human services or the educational or experiential equivalent in the field of aging or physical disability.

Candidates must have expertise in:
1. Long-term services;
2. Consumer-oriented public policy advocacy;
3. Leadership and program management skills;
4. Negotiation and problem solving skills.  

Skills required include excellent verbal and written communication, initiative and self-direction, strong organizational skills, ability to relate to others, and knowledge of, or ability to master, computer software programs associated with the position. Local travel within the district is frequent and occasional travel out of district is required.

2.  HUMAN SERVICES DIVISION: Area Agency on Aging & Independent Living
     Kentucky Homecare Program Case Manager
     Posted: February 7, 2017

The Purchase Area Development District is seeking an individual to work with senior adults as a Case Manager in the Kentucky Homecare program.  The Homecare Case Manager is responsible for assessment and on-going case management functions for older individuals living within the eight counties of the Purchase District. 

The ideal candidate will have one of the following:
1. Have a Bachelor’s Degree in Health or Human Services with
a) One year experience in health or human services. 
     or b) educational or experiential equivalent in the field of aging or physical disabilities.  
2. Be a currently licensed RN who has at least two years of experience as a professional nurse in the field of aging or physical disabilities. 
3. Have a Master’s degree which serves as a substitute for the experience required above.

Responsibilities include assessing consumers for eligibility, contacting consumers monthly by phone or in person, providing information and referral to consumers regarding other services, working cooperatively with partner agencies, adhering to all state regulations guidelines and policy and procedure requirements of the program, and completing all necessary paperwork and electronic data entry required, etc. Skills needed include excellent verbal and written communication, self-direction and excellent organizational skills, efficiency and attention to detail, and the ability to relate appropriately to consumers and other professionals.  Knowledge of or ability to learn and effectively utilize program related computer software required.

3.  HUMAN SERVICES DIVISION: Area Agency on Aging & Independent Living
     Participant Directed Services Case Manager / Service Advisor
     Posted: January 9, 2017

Due to Program Expansion, the Purchase Area Development District is seeking an individual to work with Medicaid eligible persons in the state Participant Directed Services (PDS) program.  The Service Advisor (Case Manager) is responsible for person-centered planning and on-going case management functions for residents in the eight county Purchase District.

The ideal candidate must meet one of the following: 1. Have a bachelor’s degree in a health or human services field from an accredited college or university. With at least 1 year of experience in a health or human services field, or the educational or experiential equivalent in the field of aging or disability. 2. Be an RN who has at least two years of experience as a professional nurse in the field of aging or disabilities, or 3. Have a master’s degree in a health or human services field from an accredited college or university.

Responsibilities include working with consumers and/or their representatives to assure compliance with program regulations, and visiting each consumer monthly.   Skills needed include excellent verbal and written communication, self-direction, excellent organizational skills, efficiency and ability to relate to other professionals.  A working knowledge of the Medicaid Waiver and/or Case/Care Management experience is a plus.
