Case Manager / Service Worker
Participant Directed Services: Area Agency on Aging & Independent Living

The Purchase Area Development District is seeking an individual to work with Medicaid eligible persons in the state Participant Directed Services (PDS) program. The Service Advisor (Case Manager) is responsible for on-going case management functions for residents in the eight county Purchase District.

The ideal candidate will have a Bachelor’s Degree or higher in a health or human services field and have at least:
1)    One year of experience in a health or human services field;
2)    Or the educational or experiential equivalent in the field of aging or disabilities.
3)    Or, be a registered nurse who has at least two (2) years of experience as a professional nurse in the field of aging or disabilities.
4)    Or, have a Master’s degree in a health or human services field.

Responsibilities include working with consumers and/or their representatives to assure compliance with program regulations, and visiting each consumer monthly. Skills needed include excellent verbal and written communication, self-direction, excellent organizational skills, efficiency, ability to relate to other professionals, and well developed computer skills. A working knowledge of the Medicaid Waiver and/or Case/Care Management experience is a plus. The PADD is an EOE/AA/ADA employer.

Part-Time Warehouse Worker
Commodity & Food Bank Programs

The Purchase Area Development District has immediate for a part-time warehouse worker for its Commodity & Food Bank program. This position is located in Mayfield, Kentucky and must provide the following duties: 

1)   Operation of a forklift and pallet jack 
2)   Unload all donations, weigh all items per guidelines and store items by category in their designated areas. 
3)   Must be able to lift and move food boxes containing 50+ lbs. of food. 
4)   Log weights into a weekly poundage report log. 
5)   Pull and palletize orders as they come in from Food Pantries. 
6)   Unload government commodity trucks when they arrive at the warehouse.
7)   Help keep a maintenance record for all warehouse equipment including the trucks. 
8)   Keep warehouse logs up to date. 
9)   Keep the warehouse in order and clean for periodic inspections from Health Department and other government agencies. 
10) Other duties as necessary. 

All applicants will need to possess good communication skills, the ability to work on several different projects at once and prioritize that work, and be a self-starter.
